Your privacy is very important to us. Accordingly, we have developed this Policy in order for you to understand how we collect, use, communicate, disclose and make use of personal information. The following outlines our Privacy Policy.

What is a Privacy Policy?

Under data protection law (General Data Protection Regulations GDPR May 2018) you, as a client of Creative Chiropractic Ltd, have specific rights. To communicate these rights to you in a clear and concise manner, we are providing you with this privacy notice.

Data Protection Officer

As we record and use sensitive data we take the protection of this data very seriously, observing high standards of confidentiality. We have appointed a Data Protection Officer, Business Director Linda I’Anson, who is your first point of contact for any matters regarding personal data. The Personal Data We Process and What We Do with It

Personal data is information about you which identifies you, such as your name, contact details and medical information.

The personal information you give us may also include relevant sensitive data about your physical and mental health, current medication, ailments, diseases or disabilities plus any other relevant information. We use this information to provide our services to you.

We only share your personal data with your explicit written consent, where, for example we need to contact a third party and provide them with your contact details and relevant medical information in order to refer you for an appointment such as an MRI.  Likewise, where you request or have agreed, to provide relevant information to your GP or to your insurance company who may contribute to the cost of treatment received at this Practice.

Separate to the above, and in order to provide our clients with efficient, accurate and informative health information, we use the services of Campaign Monitor solely for the processing of our periodic health newsletters and birthday vouchers via email to our clients.

Under GDPR regulations, Campaign Monitor is considered a ‘third party data processor’ who, for this specific purpose, are only provided with name, email address and date of birth and do not share these details with any other organisation or individual.

We never disclose or sell your personal information to other organisations or individuals.

Retaining Your Personal Data

Whilst you are a client of Creative Chiropractic Ltd we will continue to store and use your personal data. If you are a patient, we will retain your personal data for a period no longer than 8 years from the date of your last appointment, (in the case of a minor, up to 8 years from their 18th birthday).  Limited information will be retained within our accounts systems indefinitely to maintain the integrity of the data, this does not contain personal identifiable data.

Your Rights

As we process your personal data, you have certain rights. These are:
– A right of access
You may request a copy of your data at any time. Please make such a request in writing or by email to the Data Protection Officer, whose details are shown above. Please provide the following information: your name, address, telephone number, email address and details of the information you require.

– A right of rectification
If you believe any of the personal data we hold on you is inaccurate or incomplete, please contact us directly and any necessary corrections to your data will be made without undue delay.

– A right of erasure
If you believe we should erase your data, please contact the Data Protection Officer, whose details are shown above.

– A right to restrict processing
If you wish us to stop storing or using your data, please contact the Data Protection Officer, whose details are shown above.

Where you have provided explicit consent for us to use your data you have a right to withdraw this consent at any time.

Data Breaches

Should your personal data that we control be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, we will contact you without delay. Our Data Protection Officer will explain to you the nature of the breach and the steps we are taking to deal with it.

Should You Wish to Complain

You can contact the Information Commissioners Office (ICO) via their website: www.ico.org.uk or by phone on 0303 123 1113 should you wish to make a complaint about the way we are processing your personal data.

Automated Decision Making and Profiling

We do not use any system which uses automated decision making or profiling in respect of your personal data.
