For the last option, if you tick it, you will see it offers a dropdown with further sub-options. This could be added to any of the options but it does complicate things a bit and might put people off, just having one at the end if need be keeps it all looking neat and not over complicated. It’s just to give you an idea of what the form can do.
You can tick as many options as you like, it’s not a ‘pick one’ thing
The replies are currently set to go to your Linda@ - can be changed
Replies come through anonymously ie doesn’t show the email address, as promised on the email (GDPR)
We could give them the option to add contact details in the comments box if they want to
You may want to think about the fact that patients sometimes reply to emails we send from SqSp to us at Reception, so Reception team may see the email. Hence thinking about referring to ‘treatment’ although we control which database it is sent to. Just a thought.
